Two different options which include preventive and diagnostic treatment, extensive and comprehensive dental treatment and orthodontia (for dependent children).

Improve vision health through a routine eye exam and eye wear savings.

Higher coverage for services provided at Madonna if enrolled in Madonna Health plan (radiology, lab, physical therapy, etc.)

Employees may set aside money using pre-tax dollars for unreimbursed medical, dental and vision expenses up to IRS limit.

Eligible full time employees who participate receive a benefit of 60% of base weekly salary if unable to work due to a disabling injury or illness after 14 day waiting period.
