Rehabilitation Day Program: Outcomes

WHO WE SERVED

During the fiscal year July 1, 2006 through June 30, 2007 Madonna's RDP admitted 124 new patients. Of these 28% were referred from outside referral sources and 72% came from Madonna's inpatient programs, as part of the patient's continuum of care. Patients had the following types of conditions:

OUTCOMES

Madonna's RDP utilizes several measurements to assess the outcomes of persons served. There are no national benchmarks for rehabilitation day programs. The outcomes described below are outcomes for all RDP patients discharged between July 1, 2006 and June 30, 2007.

Patients made an average FAM** gain of 21 points in an average of 22 day length of stay, with an average overall functional level, measured by FAM** of 181 of possible 210 (or an average score of 6, "modified independent") at discharge. 100% of patients rated their overall experience as "Excellent" or "Good" and 81% strongly agreed they would recommend the program to family or friends.

* FAM is a 7-point tool used by many comprehensive outpatient providers throughout the nation to assess a patient's function. It includes the 18 areas identified in the FIM, including self-care, bowel and bladder function, transfers, locomotion, communication and cognition, plus an additional 12 areas including such things as Employability, Community Re-Entry, and Car Transfers. Maximum score on FAM is 210.
