Levels of Care
Transitional Care Unit: OverviewThe Transitional Care Unit (TCU) at Madonna Rehabilitation Hospital is an environment of hope and healing. The TCU serves people with orthopedic and/or medically complex diagnoses, helping them recover after acute care hospitalization and regain the skills needed to return to home or other settings of lower intensity care. Madonna's TCU provides a holistic approach that focuses on the physical, psychological and spiritual needs of the person and their family. TCU residents enjoy the therapeutic benefits of aromatherapy, music and gentle massage to help speed healing, reduce stress and increase overall well-being. Licensed as a skilled nursing facility and accredited by CARF, the Rehabilitation Accreditation Commission, TCU is a 21-bed unit with 11 private rooms. TCU residents have access to Madonna's exceptional medical rehabilitation services and nursing care, as well as a state-of-the art therapy gym, an expanded activities area and recently renovated patient rooms. To be eligible for admission to the TCU, the patient must have a daily skilled nursing or skilled rehabilitation need which requires medical services, such as:
Patients on the TCU at Madonna receive thereapy to meet individual needs five days per week, as well as skilled nursing care. While Madonna follows the same transdisciplinary rehabilitation approach described in the ARU section, patients on TCU may require a much lower intensity of rehabilitation due to medical needs or other factors.
