Transitional Care Unit Fast FAQs

During the fiscal year July 1, 2008 through June 30, 2009 Madonna Rehabilitation Hospital discharged a total of 234 patients on its Transitional Care Unit (TCU).

Diagnosis: A breakdown of discharges by diagnosis is illustrated below:

Age: Individuals ranged in age from 20 to 99 years old, with 24 people who were age 55 or younger.

OUTCOMES

Madonna Rehabilitation Hospital utilizes several measurements to assess the outcomes of persons served on its Transitional Care Unit. There are no sub-acute specific benchmarks available for national comparisons. The outcomes described below are combined outcomes for all patients discharged from Madonna's TCU between July 1, 2010 and June 30, 2011.

• 89.2% of patients discharged to community settings after an average length of stay of 14 days.
• On average, people with orthopedic injuries met 86% of their goals last year.
• 97.3% of patients discharged from TCU rated their overall experience as "Excellent" or "Good."
• 98.6% of patients who responded to a telephone survey completed 3 months after discharge said they had maintained the gains they made during their stay at Madonna.
