Long Term Care

The Woods House is an innovative program of Madonna Rehabilitation Hospital designed to allow younger adults with severe disabilities maximum independence. It is designed for adults ages 19 to 59 who have severe disabilities. This original program provides a choice for younger adults living with disabilities that they have never had before -- the opportunity to direct their lives in a community setting.

Madonna's program is unlike any other in the United States. It is the only program of its kind which allows a person who is ventilator dependent, or otherwise device dependent, to direct their own care.

Woods House is located on the campus of Madonna Rehabilitation Hospital and consists of eight living units with separate storage areas and a commons area, which includes a living room, dining room, kitchen, laundry room and covered patio.

In this setting, each tenant plans his or her own daily routine and directs his or her own care. This includes decisions about when to get up, schedules for medications and laundry, activity planning, etc.

• Payment Sources

  Funding for this program is available through Medicaid waiver for those individuals who qualify. All tenants must meet the qualifications for NIFA Housing. As an individual
  begins the admission process, Madonna staff will work with him or her to identify
  available resources.
