Patients attending therapy at TherapyPlus will need to bring their referral to therapy (physician order) to their initial therapy appointment. Nebraska law provides for direct access to physical therapists; however, many insurances require a referral from a physician.
Patients also need to bring any insurance information to their initial therapy appointment.
If general health insurance is covering the cost of therapy, you are required to bring your insurance card. If you are covered by more than one insurance provider, you will need to bring insurance cards for all providers.
If your therapy is due to a worker's compensation injury, you are required to bring your worker's compensation claim number, authorization for treatment and case manager contact information.
If your therapy is due to an injury sustained in an auto accident or is going to be funded by an attorney lien, please make sure to bring that information with you, including the accident report, auto insurance information, and/or attorney information if applicable.
If you have Medicaid, you will need to provide a copy of your current Medicaid insurance card each month if your therapy goes beyond the month that you initially began therapy.
