• 243 individuals were served in Madonna’s Outpatient Stroke Rehabilitation Program.

• Most people with stroke were functioning at an independent level with or without the use of adaptive equipment at the time of discharge from outpatient services.

Functional Level (FAM) at Discharge from RDP:

• FAM gain was 19.7

• DC FAM was 176.6/210 (Modified Independence)

Patient Satisfaction:

• 100% of patients with stroke rated their overall experience at Madonna as “Excellent” or “Good.”

• 100% agreed they would recommend Madonna’s Outpatient Stroke Program to their family and friends.
