To Make a Referral

Making a decision about where to go for rehabilitation services is one of the most important decisions you will need to make in the recovery process. Many families find themselves not knowing where to begin or what questions to ask. We encourage you to use the following checklist, How to Choose a Rehabilitation Program, to help guide you through your decision making process.

We conduct on-site assessments whenever possible within 24-48 hours. Our nurse liaisons work closely with families, discharge planners/case managers and physicians to begin the rehabilitation process in acute care. In addition, we are able to meet the patient and answer any questions regarding financial and insurance questions.

When making a referral, please have the following information available so that your request can be processed efficiently:

  • Person inquiring and phone number
  • Patient name
  • Date of Birth
  • Diagnosis/condition/cause
  • Payor Source

Effective communication is a critical component of Madonna's service to clients. With every patient comes a unique support system. Families, family doctors, insurance case managers - all contribute important pieces to the rehabilitation experience. At Madonna, individual communication methods are developed to ensure that the needs of all involved are accommodated.

Families are invited to attend therapy sessions and meetings with the entire treatment team throughout the patient's stay. Case managers and social workers serve as the link to the treatment team. Whether it is weekly phone contact, email or face-to-face visits, communication is tailored to each individual's needs.

Referring physicians are key to the continuum of care. Madonna physicians and case managers will maintain communication regarding recommended follow-up, medications and other pertinent information through timely discharge summaries, bi-weekly team conference notes and other communication as appropriate.

Often, insurance company case managers assist in discharge planning, as well as coordinate benefits. Madonna case managers and social workers collaborate with them to ensure the best use of benefits. Again, each patient's needs are unique. Each participant is part of a larger team of persons who contribute to the overall experience and outcome.
