Madonna Rehabilitation Hospital medical staff assists in the recovery of our patients and families during the rehabilitation process. The patient’s primary care and referring physician are involved in the treatment plan to ensure a smooth transition back to the community. Each patient at Madonna has an attending physician who is supported by interdisciplinary treatment teams and more than 300 consulting physicians representing the full spectrum of medical specialties.

Paul L. Krabbenhoft, M.D.
Staff Physiatrist and Spinal Cord Injury Program Medical Director
Board Certified–Physical Medicine & Rehabilitation,
Subspecialty in Spinal Cord Medicine
 
Dr. Krabbenhoft completed his medical residency at the University of Minnesota and is board certified in Physical Medicine and Rehabilitation. He is also certified in the Subspecialty of Spinal Cord Injury Medicine. He is a member of the American Medical Association and the National Spinal Cord Injury Association.



 


Adam T. Kafka, M.D.
Staff Physiatrist
Board Certified–Physical Medicine & Rehabilitation

Dr. Kafka graduated with a Doctor of Medicine degree from the University of Nebraska College of Medicine. After an internship in Internal Medicine at Iowa Methodist Medical Center, he completed the Physical Medicine and Rehabilitation residency program at the University of Wisconsin. Dr Kafka is the dedicated physiatrist for AVCRH and collaborates daily with patients’ families and our pediatric hospitalists.


 



Morgan T. LaHolt, M.D.

Staff Physiatrist and Brain Injury Program Medical Director

Board Certified–Physical Medicine & Rehabilitation, Subspecialty in Brain Injury Medicine

Dr. LaHolt graduated with a Doctor of Medicine degree from the University of Nebraska College of Medicine. After an internship in Family Medicine at the University of Nebraska Medical Center, he completed the Physical Medicine and Rehabilitation residency program at the University of Missouri-Columbia and served as chief resident during his final year.





 


Ana Delgado, M.D.

Board Certified–Neurology

Dr. Delgado earned her medical doctorate from the Medical School Universidad del Valle, Cali, Colombia. She completed an internal medicine internship as well as neurology residency and fellowship in neuro rehabilitation at the University of Miami/Jackson Memorial Medical Center. Additionally, she was a research associate with the Department of Neurological Surgery/Endovascular Division at the University of Miami.
 


Jackie Tran, M.D.
Staff Physiatrist
Board Certified–Physical Medicine & Rehabilitation

Dr. Tran is a graduate of New Jersey Medical School and completed a Physical Medicine and Rehabilitation residency at Rutgers/Kessler Institute for Rehabilitation. She specializes in non-surgical management of nerve, muscle, bone, movement and pain conditions. She is also certified pre-employment medical examiner for the Department of Transportation.
 

 


Tanner C. Gates, O.D.
Staff Optometrist
Board Certified–Vision Therapy, Rehabilitative Optometry

Dr. Gates has a Doctor of Optometry degree from Nova Southeastern University in Fort Lauderdale, Florida. He completed post-doctoral training in Vision Therapy/ Rehabilitative Optometry at Appelbaum Eye Care Associates in Bethesda, Maryland. Dr. Gates has extensive experience with pediatrics and binocular vision, as well as ocular disease.
