Patient Financial Services
Madonna is dedicated to delivering quality patient care and takes a positive approach to patient billing and collections. Our Patient Financial Services department is here to help you navigate the billing and payment process efficiently. We handle complex issues related to healthcare claims and work in coordination with your case manager to provide assistance, support, and information.
Insurance Benefits
Insurance coverage varies by company, and in most cases, certain deductibles must be met to obtain benefits. It is necessary to work with Madonna and your insurance company to determine what coverage is available during your stay. Insurance benefits will be verified the week of your admission. This verification of benefits does not guarantee payment of claims. In most cases, the amount paid by the insurance company is not determined until the claim or bill is submitted. Any questions concerning insurance coverage should be directed to Patient Financial Services.
Billing
- You have the right to complete and accurate billing for the services you received during your stay at Madonna. Patient Financial Services is available to assist you with any questions concerning your bill. If you think your bill is inaccurate, please write to us at:

You can reach us by phone at 402.413.3600, email at [email protected], or visit our office located at the appropriate Madonna campus from 8 a.m. to 4:30 p.m. Monday through Friday. Your letter should include the following information and be sent within 60 days of the date of your billing statement:
- Your name and account number
- The dollar amount of the suspected error
- Describe the error and explain why you believe there is an error. If you need more information, describe the item you are unsure about.
We will investigate the suspected error. You do not have to pay these charges, and we will not take any action to collect these charges while we are investigating. However, you are still obligated to pay the charges on your bill that are not in question.
Financial Assistance Offered
At Madonna Rehabilitation Hospitals, we are committed to providing the highest quality of care. The purpose of the Madonna Rehabilitation Hospital (MRH) Financial Assistance is to provide services to indigent, low-income residents of our community who are uninsured or underinsured and do not have adequate financial resources to pay for necessary healthcare services provided by MRH. This information is a summary of the MRH policy.

Eligibility Requirements and Assistance Offered
Eligibility for financial assistance is based on multiple factors, including insurance coverage or other sources of payment, income, family size, assets, and any factors the patient or physician would like to have considered. Financial assistance is offered to patients who are uninsured or underinsured. Full financial assistance will be granted based on a patient’s ability to pay the billed charges.
Patients must fully comply with the application process, including submitting tax returns, W-2s, payroll stubs, and bank statements, as well as completing the application process for all available sources of assistance.
How to Apply for Assistance
The patient or any person involved in the care of the patient, including a family member or provider, can express financial concerns at any point during the patient’s care. The patient or responsible party will then be encouraged to complete a financial assistance application. Madonna Rehabilitation Hospital’s Financial Assistance Policy and Application are available free of charge by calling Patient Financial Services at 402.413.3600 and requesting a copy by mail or email. No More Than Amount Generally Billed (AGB)
A patient determined to be eligible for financial assistance will not be charged more than the amounts generally billed for medically necessary care to patients who have insurance for such care.

Good Faith Estimate
- Good Faith Estimate Rights for Patients who are Uninsured or Choosing not to Use Insurance:
You have the right to receive a Good Faith Estimate explaining how much your medical care will cost if you are uninsured or choosing not to file a claim with your insurance. Under the law, healthcare providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
- Make sure your healthcare provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service or item.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate.

No Surprises Act
When you receive emergency care or treatment from an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected from surprise billing or balance billing.
