Health Plan
Three options offered through Blue Cross Blue Shield of Nebraska. Pharmacy benefits included.
High Deductible Health Plan (HDHP)
The HDHP offers a lower premium and a substantially higher deductible than the PPO plan. Instead of co-pays, you pay the entire cost of the service, prescription, etc. and 100% of these costs count toward your annual deductible. A 20% coinsurance amount applies once you meet your deductible until you reach your maximum out-of-pocket and then the plan pays at 100%. HDHP participants are enrolled in the Health Savings Account (HSA) with Madonna making an annual contribution upon enrollment. Employees may also make a pre-tax contribution to the HSA.
Preferred Provider Organization (PPO)
The PPO offers higher premiums, lower deductibles and co-pays for services, procedures, and prescriptions. A 20% coinsurance amount applies once you meet your deductible until you reach your maximum out- of-pocket and then the plan pays at 100%.
Affordable Care Act (ACA)
This plan is available only to part-time employees and meets the requirements of the Affordable Care Act. There is no HSA option with this plan.

Dental Plan
Two different options which include preventive and diagnostic treatment, extensive and comprehensive dental treatment and orthodontia (for dependent children).

Vision Plan
Improve vision health through a routine eye exam and eye wear savings.

Madonna Services
Higher coverage for services provided at Madonna if enrolled in PPO option. These services include radiology, lab, physical therapy, etc.

Flexible Spending Accounts
Health Care Flexible Spending Account (HCFSA)
Employees may set aside money using pre-tax dollars for unreimbursed medical, dental and vision expenses up to IRS limit HCFSA.
Limited Purpose Flexible Spending Account (LPFSA)
Allows employees to set aside pre-tax dollars to be used for qualifying dental and vision expenses only. To enroll in an LPFSA , you must elect coverage under Madonna’s HDHP and HSA.
Dependent Care Flexible Spending Account (DCFSA)
Employees may set aside money using pre-tax dollars to pay for dependent care costs (i.e. day care charges). Enrollment is not tied to any health plan enrollment and can be utilized whether enrolled in PPO, HDHP, or ACA option.

Health Savings Accounts
Health Savings Account (HSA)
An HSA is a triple tax-advantaged account that allows employees to set aside funds to pay for qualified medical, dental, and vision expenses (up to current IRS limits). Madonna will also contribute to your HSA if you are enrolled in our HDHP. Enrollment in the HSA is required when you elect Madonna’s HDHP.

Short & Long Term Disability
Short Term Disability
Eligible full time employees who participate receive a benefit of 60% of base weekly salary if unable to work due to a disabling injury or illness after 14 day waiting period.

Long Term Disability
Eligible full time employees receive a benefit at no cost of 50% of base monthly salary if unable to work due to a disabling injury/illness after 90 day waiting period. Additional 66 2/3% coverage may be purchased at employee expense.

Life Insurance
Employee Life Insurance
Eligible full time employees receive one times annual base salary at no cost. Employees may purchase supplemental life insurance in increments of their salary (1 times up to 4 times salary). Accidental Death and Dismemberment is included in employee life coverage.

Dependent Life Insurance
Spouse and dependent children coverage is available for purchase.

Wellness Program
Madonna’s Occupational Health & Wellness program provides wellness activities, programming, seminars and challenges throughout the year. Wellness Health Screenings are held annually at no cost to employees.

Paid Time Benefits (PTB)
Combines vacation and paid leave including holidays, sick leave, etc. providing flexibility in using earned time off.

Eligible employees and their immediate family members receive free short term confidential counselling and referral to other resources through Best Care EAP.

Tuition Reimbursement
Eligible employees enrolled in undergraduate LPN, RN or RT programs may receive $3,000 to $5,250 per year. Eligible employees not enrolled in these programs may receive up to $3,000 per year for other undergraduate degree paths. Eligible employees enrolled in a Master of Science in Nursing (MSN) program may receive $3,750 to $7,500 per year.

Medical Fitness Facility – Madonna Wellness Club (Lincoln)
Employees, spouses and dependent children may join Wellness Club, Madonna’s state of the art fitness and wellness facility, at a discount.

Employee gift is given every five years beginning with your fifth anniversary.

Differentials are paid to most MRH employees regularly scheduled to work evening, night and weekend shifts.
