We have made the referral admission process very easy for you. Just complete this form and one of our team members will be in touch with you within 24 hours. Please be sure to include your name, phone number and how we can assist you.

2. Onsite assessment

Our nurse liaisons work closely with physicians and hospital staff to make the transition to Madonna as smooth as possible. When able, we will conduct an on-site assessment to gather clinical information, meet the patient and family and complete a clinical assessment to determine the most appropriate level of care and program.

3. Get to know Madonna

We invite you to tour our facilities, meet our medical staff and learn more about the rehab experience.

Making the Right Choice

There is a difference between a rehabilitation hospital and a rehabilitation unit. When searching for a complex medical program or a acute rehabilitation program for your loved one, it’s important to know what’s important to ask. 

At Madonna, we take a transdisciplinary approach to rehabilitation. It’s more than therapy, it’s about helping people resume their life roles by focusing on the whole person.

Insurance Partners

Our Admissions Team

We believe that rehabilitation starts in the acute care hospital. At Madonna, we have a team of nurses who are committed to talking with each of our patients about the importance of making a good decision about what to look for when choosing a rehabilitation hospital. A nurse liaison is a registered nurse who will coordinate a pre-admission assessment for potential patients to our programs. This assessment may include on-site evaluation of the patient’s medical and rehabilitation needs, interview with patient and family, as well as discussion and coordination of patient needs with physicians, nurses and insurance personnel. The nurse liaison will synthesize this information to make a determination of the patient’s eligibility for admission to one of Madonna’s programs, as well as educate the patient and family about the rehabilitation process and what to expect. The nurse liaison will then coordinate the transfer of the patient and his or her medical records to Madonna, as well as communicate any special needs to our staff.
