Medical Records Apps
Fast Healthcare Interoperability Resources (FHIR) Revision 4 (R4) is a federal communication standard that allows patients to access their medical records electronically. The main benefit of using a FHIR enabled app is that it allows a patient to access their medical record data across multiple provider or hospital systems. There is no charge for signing up or using these apps.
Complete the process in three steps.
Step 1: Confirm your email address
- If you received an email from “Allscripts Open Services”, please allow at least 20 minutes before moving on to Step 2.
- If you have not received an email or are unsure, please click here to complete the on-line FHIR R4 App Request form. In the message section, click the box next to email/invitation. You should expect to receive an email within 3 business days.
Step 2: Registration
- The email you receive from Allscripts Open Services will include an invitation code. The invitation code is only valid for 24 hours. Follow the instructions within the email to complete the registration process.
Step 3: Select FHIR R4 App
Click on the link to the available FHIR R4 App below and follow the prompts within the App. You will be looking for “Gather Records”. In doing so, you may see a list of facilities and providers to select from. If you do not see Madonna listed, you will need to search for it.
Below is a list of confirmed FHIR R4-enabled apps that work with our system. We will add additional apps as verification processes are completed.

If you are requesting medical records on behalf of a patient or client, you cannot use the above-listed apps and must fax records requests.
