Madonna celebrates National Case Management Week

At Madonna Rehabilitation Hospitals, Collaboration is among our core values, and our Case Management team exemplifies this value every day. They are charged with facilitating communication between physicians, insurance companies, nurse liaisons, hospital care teams and families to provide the outstanding care that Madonna is known for throughout the country.  This week is designated as national Case Management Week to recognize these dedicated professionals and their contribution to the patient experience.

“When I think of Case Management, their advocacy and their ability to develop meaningful relationships with the patient and family comes to mind. They are ultimately responsible for the successful discharge destination, finding community resources and working with insurance companies to continue payment for our plan of care. Through all kinds of difficult scenarios, they maintain patient rights, confidentiality and really good humor, even with challenging cases. We love and respect our case management team and the work they do!” said Marilyn Viehl, Madonna Rehabilitation Hospital Omaha Administrator.

The dedicated professionals on Madonna’s Case Management team are master liaisons. It is their job to help educate and advocate for the services based on the patients’ needs to help them recover and to help them through their rehabilitation process, including communication with insurance providers.

Case Management also educates and empowers patients and their families by providing them with resources to help them be successful after they leave Madonna. They are very resourceful and have a wealth of knowledge to educate patients and their family members as they prepare to discharge to a community setting or another health care facility. They ensure that patient families feel confident, are equipped to care for their loved one and are empowered with the tools available to them. They help to ensure the patient experience is as seamless as possible so that patients continue to thrive well beyond their stay at Madonna.

Susan Koerber, director of Case Management Hospital programs, sums it up by saying, “I am so proud to be a part of the Case Management departments for the Madonna Rehabilitation Specialty Hospitals at both the Lincoln and Omaha campuses. It is truly an honor to work with so many passionate and talented individuals that care so much for our patients and their outcomes.”
